chronic inflammatory bowel diseases

MabDesign’s market analyses give you a monthly overview of market dynamics and projects in development for a given indication or technology, to help you better understand the constantly evolving market of biologics and biomanufacturing. This month, an overview of the market and projects currently under development for the treatment of inflammatory bowel diseases (IBD).

Inflammatory bowel diseases (IBD), which mainly encompass Crohn’s disease and ulcerative colitis, are chronic immune-mediated disorders of the gastrointestinal tract. With an incidence of approximately 4.97 cases per 100,000 people globally, more than 300,000 new cases were reported in 2021, alongside around 42,000 IBD-related deaths the same year. The steady rise in incidence ,particularly in newly industrialized regions, combined with the chronic and relapsing nature of the disease, places IBD among the conditions with a significant and growing socio-economic burden.

Pipeline

The global IBD therapeutic market reached approximately 22 billion USD in 2025, with a compound annual growth rate (CAGR) of 5.9% between 2024 and 2031, reflecting a dynamic and innovation-driven landscape.
Currently, the market is overwhelmingly dominated by small molecules, which account for 96.1% of marketed products, while biologics represent only 3.9%.

However, a profound shift toward biologics is emerging: within the development pipeline, biologics now represent 56.8% of ongoing projects, versus 43.2% for small molecules, highlighting a move toward more targeted and disease-modifying therapeutic approaches. In 2025, the innovation momentum remains strong, with 203 new deals announced and 58 new biologics entering the pipeline.

Focus on the biologics pipeline

The biologics segment shows an increasing diversity of modalities:

  • 41.2% are monoclonal antibodies,
  • 8.2% are cell therapies,
  • 5.5% fusion proteins,
  • 5.5% recombinant proteins,
  • 4% peptides,
  • 4% recombinant peptides,
  • and 2.7% gene-modified cell therapies.

This diversity reflects an ambition to address persistent unmet needs, particularly regarding the achievement of sustained remission, overcoming primary and secondary resistance to current therapies, and adapting treatments to the heterogeneous clinical profiles of IBD patients.
